The story follows two American tourists, Lindsay and Jenny, who are traveling through Germany. After their car breaks down in the middle of a isolated forest during a storm, they seek help at a nearby villa.
The film then follows Heiter's cold and clinical execution of this deranged project. The victims wake up to find themselves stitched together in the order of Katsuro (mouth to Lindsay's anus), Lindsay (mouth to Jenny's anus), and Jenny at the front.
